"DO YOU KNOW A REGISTERED IMAGING PROFESSIONAL WHO HAS MADE A DIFFERENCE?"
It is time that we recognize Imaging Professionals who have made a significant difference to the imaging profession and in the lives of patients, peers, and the community. The NCSRT, Inc. would like you to nominate those special Imaging Professionals who continually promote our profession.
Any individual, client/patient, or group may nominate an Imaging Professional who is currently working in North Carolina. To nominate an Imaging Professional:
- You may get the current application at the end of this page or here.
- You may either fill it out by hand or print out the application. We strongly prefer typed applications. If filling out by hand, please print the information. Sections that are not legible and cannot be read will not be scored, thus resulting with a lower score for the nominee.
- How you present your candidate and the information you provide affects the scoring. So do your best to make your nominee shine.
- Nomination documents will not be accepted by fax transmission. Postmark deadline to send all documents by CERTIFIED MAIL is July 15, 2008 to:

- Recipients will be notified by mail.
- Awards ceremony will be held in Oct. at the 2008 NCSRT, Inc. annual conference. The recipients name will be posted on the website after the annual conference.
Additional information:
- Members involved in the committee are not eligible.
- Previous Imaging Professionals for Excellence recipients are not eligible.
- Nomination letters will be reviewed (blind review) by a panel of judges who will not have access to the recipient’s personal information.
Tips for writing a Nomination
- Nominate an Imaging Professional who has excelled in his or her field.
- Complete all sections of the application. Resumes / curriculum vitae’s are NOT accepted.
- Try to obtain the correct personal information of the nominee on page 1, applications with incorrect information maybe rejected.
- Complete pages 2, 3 and 4 with clear and concise explanations (use examples).
- Each application is scored separately, therefore; it is appropriate to nominate a qualified person even if you think someone else is going to nominate them.
- Send your applications as early as possible. Only applications that are postmarked by July 15, 2008 will be accepted.
